Abordaje integral de los cuidados espirituales en salud en atención primaria: revisión bibliográfica

Resumen
Propósito: Recopilar la información existente sobre el abordaje de los CES en la atención primaria, el rol del médico como proveedor de cuidados espirituales, las técnicas de evaluación y abordaje y las posibles barreras o facilitadores. Metodología: Esta es una revisión bibliográfica sobre las publicaciones realizadas en los últimos 10 años acerca de las herramientas más utilizadas en la atención primaria para los cuidados espirituales. Se utilizaron bases de datos científicas: PubMed, ScienceDirect y Sagepub. Siguiendo los criterios de inclusión y exclusión se realizó un estudio en tres fases principales: (a) lectura de títulos y abstracts, (b) tabulación de tablas y (c) análisis de los artículos seleccionados. Resultados: Un total de 989 artículos fueron identificados, 822 excluidos por no apegarse al tema o estar duplicados. De los 167 artículos, 115 se excluyeron por no cumplir los criterios de inclusión. Un total de 52 artículos fueron sometidos al análisis. Conclusiones: Existen estrategias de intervención que pueden ser utilizadas desde la atención primaria, en población variada, individuos sanos y diversos problemas de salud. Se concluye que los instrumentos validados más utilizados de acuerdo con la clasificación utilizada son: FICA, DSES, SIWB, SpNQ. Se pueden implementar programas que integren los CES con los objetivos terapéuticos (autocuidado del diabético, cifras tensionales, control de peso y dolor) y adaptarlas a la atención primaria. Se establecen las principales barreras y facilitadores para la prestación de Cuidados Espirituales en Salud.
Inglés: Purpose: This study aims to compile the existing information about spiritual care in primary healthcare, the physician’s role as a spiritual care provider, possible barriers and facilitators, evaluation and intervention techniques. Methodology: This study reviews spiritual care literature published over the last 10 years, regarding spiritual care tools most frequently used in primary healthcare. A systematic literature search was conducted using: PubMed, ScienceDirect and Sagepub. Following the inclusion and exclusion criteria, this study consisted of three main phases: (a) reading of titles and abstracts, (b) tabulation of tables and (c) analysis of the selected articles. Results: A total of 989 articles were identified, 822 excluded for not adhering to the topic or being duplicates. Of the remaining 167 articles, 115 were excluded for not meeting the inclusion criteria. A total of 52 articles were submitted to the analysis. Conclusions: There are spiritual care intervention strategies that can be used from primary care, in a varied population, ranging from healthy individuals to those facing diverse health problems. It is concluded that the validated instruments most utilized according to the classification used are: FICA, DSES, SIWB, SpNQ. Programs that integrate Spiritual Healthcare with therapeutic objectives can be implemented (self-care of the diabetic patient, blood pressure figures, weight control and pain) and adapted to primary care. The main barriers and facilitators for the provision of Spiritual Care are established.
